STEP 1:

You will need to enroll or re-enroll if you meet any of the following

  • You don't have a plan with Solstice Benefits and want to create a NEW plan.

  • You are currently enrolled with Solstice Benefits/ Starmark Benefits and want to change your plan.

  • You are currently enrolled with Solstice Benefits/ Starmark Benefits and want to add or delete a spouse or dependents.

Please download the Schedule of Benefits or Program Information to obtain detail information on each plan.

You can now register online using the link to register. If you register online you don't have to print and fax the DOE 70/Pre-Paid Enrollment Forms. For the manual registration: You will need to download, print and complete your DOE 70 and Enrollment forms then fax them to (954)370-1737
